--“Revocation; Fine of $50,000 stayed for 60 days. Following a Show Cause Hearing, the Board found that the physician violated the Consent Order of June 1, 2010.”  

-- Suspension for a minimum of 3 years with all but 18 months stayed and until completion of terms and conditions; Probation for 3 years subject to terms and conditions. The Board found the physician guilty of failing to cooperate with the Board’s investigation, to provide details of a patient’s medical record upon proper request, and guilty of abandoning a patient.”

--“On June 1, 2010 Dr. Soudah entered into another CONSENT ORDER to resolve charges of unprofessional conduct in the practice of medicine, patient abandonment, failure to provide patients with details of their medical records and failure to cooperate with a lawful investigation conducted by the Board.”

--“According to Dr. Soudah’s acquaintance, he did not tell his patients he was leaving the country and did not inform them as to how to obtain their medical records…The acquaintance stated that she subsequently went to clean up Dr. Soudah’s office and while there she found approximately 2000 active patient records, some of which contained pap smears that Dr. Soudah never reviewed.
