--“The Board took emergency action based on the physician’s previous history with the board, his may 2008 arrest for possession of crack cocaine and drug paraphernalia and subsequent charge for possession of CDS and drug paraphernalia; his September 2008 arrest and charge with possession of CDS and other investigatory information that in 2008 the physician regularly purchased and used crack cocaine.”
--“Witness A reported that thereafter on average three to four times a week the Respondent would give her money that she would then use to purchase an “eight ball” of cocaine. She reports that she and the Respondent would use the drugs at the Respondent’s residence where he resided with his two children, who were usually present in the downstairs portion of the house while she and the Respondent used drugs in an upstairs bedroom. She reported that the Respondent would usually spend approximately $150.00 on each purchase of crack cocaine, but at times he spent as much as $300.00 to 400.00 per day.”
