--“The Boards investigation determined the following: The Respondent is addicted to alcohol and opiates. The Respondent’s substance abuse history involves the recreational use of illicit controlled substances, including cocaine, opiates, hallucinogenic drugs and marijuana. On multiple occasions, the Respondent misappropriated the identity of a professional associate to write prescriptions for narcotics, benzodiazepines and other controlled substances in the name of a family member, which he then diverted for his own use/abuse. The Respondent practiced medicine while under the influence of controlled substances and/or practiced medicine while in withdrawal from the use of those controlled substances.”
