BALTIMORE - Health care officials, elected leaders and community members will come together on March 11 to launch the work of the West Baltimore Primary Care Access Collaborative. It's a new partnership focused on strengthening the primary care delivery system in West Baltimore.
The creation of the Collaborative marks a major milestone for the West Baltimore community, which has suffered in recent years from a lack of primary care and care coordination.
More than a dozen institutions have joined the Collaborative to work together to strengthen the health care system, improve access to care and reduce persistent and profound health disparities in a large section of West Baltimore.
A formal launch will take place at 11am on Monday March 11th at the Talon Center at Coppin State University.
