The National Transportation Safety Board released the cause of last year's Medevac helicopter crash.
Investigators say several factors contribiuted to the crash, including lack of pilot training and oversight of the program. The board did not find any signs of mechanical failure.
NTSB board member Robert Sumwalt says, "It raises concerns about what's going on in the organization."
